Information Collection Statement

Personal information you provide in this form is collected pursuant to section 26 of the Freedom of Information and Protection of Privacy Act.

This information will be used for the purpose of facility bookings in the Regional District of Central Kootenay Community Services.

Insurance Requirements

All low, medium, and high risk rentals require proof of insurance upon booking.

Special Events (banquets, festivals, tournaments, etc) require proof of insurance 30 days prior to the event.

If proof of insurance is not provided by the appropriate deadline, the rental will be cancelled.

Please ensure the name insured on the insurance certificate is the same as the name used on the RDCK Facility Rental Agreement.

Schools are not required to provide proof of insurance.

COVID-19 SAFETY PLANS

In accordance with the Gathering and Events Order issued by the Medical Health Officer for Interior Health Authority, facility rental organizers of non-vaccinated events are required to provide a COVID-19 Safety Plan. RDCK has created a COVID-19 Safety Plan Questionnaire that can be used as an appropriate Safety Plan for events. All Safety Plans are subject to review by RDCK Staff before considered complete. Please be as detailed and thorough as possible. If you have an existing Safety Plan or supporting documents, there is an opportunity to upload them within the questionnaire.
